Republic of the Philippines PROVINCE OF LA UNION BIDS AND AWARDS COMMITTEE REQUEST FOR QUOTATION (RFQ) The Provincial Government of La Union (PGLU), through the PGLU Bids and Awards Committee (BAC), intends to apply the sum of ₱1,900,000.00 to procure the Improvements of the Provincial Accounting Office/ INFRA-26-00-GF-002 which will be undertaken in accordance with Section 34 Small Value Procurement of the Implementing Rules and Regulations of RA 12009. Interested Suppliers are required to submit the following documents: a) Mayor’s/Business Permit; b) PhilGEPS Registration Number (Platinum Membership may be submitted in lieu of the Mayor’s Business Permit and PhilGEPS Registration Number); c) PCAB License; d) Quotation Form, Bill of Quantities and Cash Flow by Quarters; and e) Notarized Omnibus Sworn Statement. f) Tax Clearance g) Income/Business Tax Return h) BIR Certificate of Registration Award of contract shall be made to the lowest quotation which complies with the minimum requirements as specified in the Implementing Rules and Regulations (IRR) of Republic Act (RA) No. 12009. Suppliers who offer or quote above the Approved Budget for the Contract (ABC) shall be automatically disqualified. Any interlineations, erasures or overwriting shall be valid only if signed or initialed by the bidder or his/her duly authorized representative/s. Bids must be duly received by the BAC Secretariat through manual submission at the BAC Office, Provincial Capitol Building, City of San Fernando, La Union on or before 9:59 AM, June 4, 2026.
