Province of Cebu Municipality of Balamban OFFICE OF THE BIDS and AWARDS COMMITTEE Telefax: (032) 345 – 8220 INVITATION TO BID FOR MEDICAL EMERGENCY SUPPLIES (MDRRMO) 1. The Municipality of Balamban, through the Mayor’s Office – 5% DRRM Fund 70% Medical & Dental Supplies Expense, intends to apply the sum of TWO HUNDRED NINETY NINE THOUSAND SIX HUNDRED EIGHTY PESOS ONLY (₱ 299,680.00) being the Approved Budget for the Contract (ABC) to payments under the contract for MEDICAL EMERGENCY SUPPLIES (MDRRMO). Bids received in excess of the ABC shall be automatically rejected at bid opening. 2. The Municipality of Balamban, now invites bids for MEDICAL EMERGENCY SUPPLIES (MDRRMO). Delivery of Goods is required 30 days after the issuance of the Notice of Award. Bidders should have completed, at least 5 years from the date of submission and receipt of bids, contracts similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instruction to Bidders. 3. Bidding will be conducted through open competitive bidding procedures using a non-discretionary “pass/fail” criterion as specified in the IRR of RA No. 12009. Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA 5183. 4. Interested Bidders may obtain further information from the Municipality of Balamban and inspect the Bidding Documents at the address given below from 8:00 AM- 12:00 AM and 1:00 PM- 5:00 PM, Monday to Friday only. 5.
