Conduct of Annual Medical Exam (Lot 1) and Drug Test (Lot 2) for PEZA-BCEZ and Cluster 3 Employees Rebid #1

LOT 1: ANNUAL MEDICAL EXAMINATION Complete Blood Count Urinalysis Fecalysis Chest X-ray Complete physical examination which includes comprehensive assessment in medical history (past, current, family, social reproductive, vaccinations), medical evaluation of results, vital signs taking, (includes BMI determination) with visual acuity and Ishihara testing) Fasting Blood Sugar, Lipid Profile (Cholesterol, Triglyceride, HDL, LDL) and Serum Uric Acid ECG (for 30 y/o and above) Papanicolaou Smear for females 25 y/o and above PSA for males 40 y/o and above TERMS OF REFERENCE 1. All test/s or examinations, including the physical examination will be conducted for two (2) days onsite at PEZA-BCEZ Medical Clinic. Diagnostic Testing and Physical Examination will be conducted separately depending on the release of the laboratory results, i.e. Day 1- Diagnostic Testing ; Day 2- Physical Examination 2. BCEZ employees who are unable to participate during the scheduled AME shall undergo the procedures at the clinic/branch of the service provider within two (2) weeks from the scheduled testing. 3. The Service Provider must have: a. Fully functional mobile X-ray and other equipment and supplies necessary for the conduct of laboratory testing; b. Disposable vaginal speculums for the conduct of Pap Smear; c. Courteous and well-trained personnel to conduct the required tests i.e. nurse, medical technologist, and physician) d. Measures in place in compliance with the Data Privacy Act to protect the rights of PEZA employees. This shall include: i. Securing written consent to collect personal information relevant to the APE ii. Safeguarding the privacy and security of personal data collected 4. Receipt of results in duplicate copies (laboratory results and APE form) is within 20 calendar days after completion of the tests. 5. PEZA shall not be liable for breakdown/malfunctioning of any equipment and thus, liability arising from them shall be borne by the Service Provider 6. Specimens taken
