SUPPLY AND DELIVERY OF ESSENTIAL MEDICINE AND SUPPPLIES FOR THE YAKAP ENROLLED PATIENTS

The MUNICIPAL LOCAL GOVERNMENT UNIT OF DUPAX DEL SUR, through its Bids and Awards Committee (BAC), hereby invites all interested bidders/suppliers, to quote their lowest price on the project stated below, subject to the Terms of Reference (TOR) and submit the same duly signed by their authorized representative not later than 9:00 AM on October 13, 2026 at the Procurement Office 2ND NEW LGU BUILDING. Opening of sealed bids will be at 11:00 AM on the same day at, MUNICIPAL COMPOUND, DUPAX DEL SUR, NUEVA VIZCAYA. LOT NO. DESCRIPTION APPROVED BUDGET FOR THE CONTRACT (ABC) 1 ESSENTIAL MEDICINE AND SUPPPLIES 230,000.00 Bids received in excess of the ABC shall be automatically rejected at bid opening. The prescribed Request for Quotation Form may be acquired by interested Bidders at the Procurement/BAC Office upon payment of the applicable fee (Php 500.00) for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB. The Procuring Entity shall allow the bidder to present its proof of payment for the fees in person or through electronic means. TERMS OF REFERENCE The following documentary requirements shall be submitted together with the price quotation (sealed in an envelope): 1. Latest Business/Mayor’s Permit issued by the city or municipality where the principal place of business of the bidder is located; X 2. Valid PhilGEPS Registration Number/Organization ID/PhilGEPS Certificate of Registration (Red or Platinum Membership); X 3. Latest Income or Business Tax Return (for ABCs above ₱500,000.00); and X 4. Notarized Omnibus Sworn Statement using GPPB-Prescribed Format, accompanied with proof of authorization, if applicable (for ABCs above ₱200,000.00). Proof of authorization shall be a duly notarized Secretary’s Certificate, Board/Partnership Resolution, or Special Power of Attorney, whichever is applicable, in case of a corporation, partnership, or cooperative, or joint venture; or a duly notarized Special Power of Attorney in case of a sole proprietors
