The Pasay City Government – Pasay City General Hospital, through its Bids and Awards Committee, and in accordance with the provisions of RA12009 and its Implementing Rules and Regulations, intends to apply the sum of One Hundred Ninety-Nine Thousand One Hundred Sixty Pesos (Php 199,160.00) being the Approved Budget for the Contract (ABC) for the project: “Supply and delivery of Meals (Buffet & Packed), Printing of Tarpaulin and Customized T-Shirts including Photo Booth to be used for the conduct of PCSD Nurse’s Week of the Pasay City General Hospital, Patient Care Services Department” chargeable against the TRUST FUND. The Pasay City Government BAC now requests the quotation for the project: “Supply and delivery of Meals (Buffet & Packed), Printing of Tarpaulin and Customized T-Shirts including Photo Booth to be used for the conduct of PCSD Nurse’s Week of the Pasay City General Hospital, Patient Care Services Department”. Quotation received in excess of the ABC shall be automatically rejected at the opening of price quotations. Please submit your duly signed quotations/proposals not later than October 14, 2026 at 10:00am for the item described above, subject to the Terms and Conditions provided with this Request for Quotation (RFQ). A copy of your PhilGEPS Organization ID or PhilGEPS Certificate of Registration, Mayor’s / Business Permit, Income/Business Tax Return (for ABCs above ₱500,000.00), Omnibus Sworn Statement (GPPB Format, notarized, for ABCs above ₱500,000.00) and Warranty/Security Certificate (if applicable) are also required to be submitted along with your quotation/proposal.
