The following documents shall be submitted, as applicable: 1. Valid Business/Mayor's Permit; 2. PhilGEPS Registration/Certificate, when applicable; 3. Valid PCAB License, for infrastructure projects, when applicable; 4. Omnibus Sworn Statement; 5. Tax Clearance, when required; 6. Latest Income/Business Tax Return, when required; 7.
