INVITATION FOR NEGOTIATED PROCUREMENT Livelihood Support and Development Program (Food Commodities) The Local Government Unit of Perez, Quezon through its Bids & Awards Committee (BAC) invites interested bidders to participate in the Livelihood Support and Development Program (Food Commodities) with an Approved Budget for the Contract (ABC) of Four Hundred Seventy-Six Thousand Four Hundred Five Pesos (Php 476,405.00) in accordance with Sec. 53.1 of the Implementing Rules and Regulations of R.A No. 9184, otherwise known as the Government Procurement Reform Act. The following eligibility and technical documents as well as the Financial Proposal Form shall be submitted on or before October 20, 2026 at 2:00 p.m. at the Office of the BAC Secretariat, Municipal Building, Perez, Quezon. a.) Authority Signatory b.) Security and Exchange Commission (SEC) / Department of Trade and Industry (DTI) Registration Certificate c.) Valid Mayors Permit d.) Valid Tax Clearance e.) Statement of Single Largest Completed Government of Private Contracts Similar in Nature and complexity within the three (3) years period f.) Statement of All On-Going and Awarded Government and Private Contracts whether similar or not similar in nature and complexity to the contract to be bid. g.) Latest Audited Financial Statements h.) Computation of Net Financial Contracting Capacity (NFCC) i.) PHILGEPS Certificate of Registration j.) Compliance with Technical Specifications k.) Omnibus Sworn Statement Opening of bids and final offer will be on October 20, 2026 at 2:00 p.m.. at EDJA Hall, Municipal Building, Perez, Quezon.
