REQUEST FOR WRITTEN PROPOSAL (NEGOTIATED PROCUREMENT-SAGIP SAKA) The Local Government of Dingras, through the Trust Fund under the Local Government Support Fund-Financial Assistance to Local Government Units (LGF-FALGU), invites FFEDIS-registered suppliers, particularly local FFEDIS-registered suppliers, to participate and negotiate with the Bids and Awards Committee (BAC) for the procurement of rice Assistance under the LGSF-FALGU with minimum requirements listed below, to wit: Procuring Entity: Local Government Unit of Dingras, Ilocos Norte Project Name: Assistance to Indigent Individuals or Families – Food Assistance/Rice Distributions Source of Fund: LGSF-FALGU (Trust Fund) Approved Budget: Five Million Pesos (Php 5,000,000.00) for the Contract The interested supplier shall deliver the required quantity of goods upon receipt of the Purchase Order and Notice to Proceed. Interested FFCE/FFEDIS-registered suppliers are invited to submit their proposal to the BAC office at Dingras Municipal Hall on or before the 21st day of October, at 2:00 late submissions shall not be accepted. a. Description: 1. Technical Criteria: 1.1 10 kilograms per sack (with printing of Bagong Pilipinas logo on the front and back of the sack, the Presidential Seal, and the LGU Seal, with printing size proportionate to the size of the sack) 1.2 8,333 sacks 1.3 Well Milled b. Eligibility Criteria 1. Must be registered in the FFEDIS. 2. Accreditation as Civil Society Organization (CSO) 3. Registration in the Registry System for Basic Sector in Agriculture (RSBSA); 4. Registration in the Fishery Registry (FishR) System; 5. Registration in the National Coconut Farmers Registry System (NCFRS); or 6. Registration or accreditation in any other DA-approved registry or accreditation system. c. Documentary Requirements: 1. Written Proposal (see attached Annex A); 2. FFEDIS Certificate of Registration or an equivalent certificate/document issued by the DA pursuant to Section 11 of the IRR of RA No. 1132
