Quotations can be submitted via fax (088-521-0022) or email (pmo@mharsmc.doh.gov.ph). 5. Quotations exceeding the Approved Budget for the Contract shall be rejected. Please see the additional list of requirements at the back. a) Mayor's Business Permit b) Sworn Affidavit of the bidder that it is not related to the Head of Procuring Entity. Please see additional requirements at the back. Terms and Condition: 1. Price quotation/s shall be valid for a period of at least sixty (60) calendar days from the date of submission. 2. Delivery period within thirty (30) calendar days from receipt of the Approved Purchase Order. 3. Awards of contract shall be made to the lowest quotation that complies with the technical specifications and other terms and conditions stated herein. 4. MHARSMC reserves the right to reject any or all offers as may be considered most advantageous to the institution.
