REQUEST FOR QUOTATION (Mode of Procurement – Small Value Procurement) The Municipal Government of Tungawan, Zamboanga Sibugay, through its Bids and Awards Committee, will undertake a Small Value Procurement for the Asisstive Devices (Wheelchairs, Crutches and Walking Stick) for PWD’s in accordance with IRR of R.A.12009, otherwise known as The New Government Procurement Act. Name of Project: Procurement of Asisstive Devices (Wheelchairs, Crutches and Walking Stick) for PWD’s ABC: Php 239,000.00 Specifications: See the attached Annex “A” for specifications Location: MSWDO Delivery Date: Within Seven (7) calendar days from receipt of Purchase Order Interested suppliers are required to submit two (2) copies of their duly accomplished Price Quotation Form (ANNEX A) on or before the dead line of submission of bids to the Bids and Awards Committee Office, 2nd Floor, LGU- Tungawan, Zamboanga Sibugay. Bidders must submit sealed quotation/s in the address given above together with the following documentary requirements: 1) Valid and Current Mayor’s/Business Permit, DTI, Certificate of Registration(BIR),Tax Clearance & Philgeps Registration; Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form.
