The provision of planned palliative nursing / healthcare assistant care covering a single patient per shift (overnight) in their usual place of residence who are in the end of life stage of their illness whose needs can be met through short episodes of care delivered through an agreed combination of days and/or evenings and/or nights as appropriate to support the needs of patients and carers. The Provider will provide registered nurses and healthcare assistants to deliver care and support for referred patients with terminal illness.

End of Life and Palliative Care Services - Inpatient Care The service will provide a ten bedded inpatient unit for terminal care, symptom control, assessment and crisis care for individuals who have a progressive terminal condition with specialist palliative care needs. The service provided covers the population of North Lincolnshire and East Riding of Yorkshire. End of Life and Palliative Care Services - Daycare The service will provide access to specialist end of life and palliative care daycare services which will include assessment, therapy, counselling and social support. The service provided covers the population of North Lincolnshire. Community Lymphoedema Service The service will provide a community lymphoedema service which supports the specialist management of individuals living with lymphoedema, lipoedema and chronic oedema aged 18 and above. The service will be delivered in line with British Lymphology Society guidelines. The service provided covers the population of North Lincolnshire. Discharge to Assess (D2A) and Community Beds The service will provide six nurse-led community beds provide short-term care for patients who are medically fit for discharge but require further assessment, therapy, or care planning. The beds support timely discharge from Scunthorpe General Hospital and enable patients to receive care in a community setting while longer-term arrangements are made.

The service will provide an End-of-Life Care Co-ordination Service that will work collaboratively across primary, community, secondary care and third sectors to facilitate choice in respect to preferred priorities for care and death. Volunteers Visitor Service The service will support carers of people living with a terminal illness through a team of trained volunteers who will undertake regular home visits to individuals with end-of-life care needs. The service will provide practical, emotional and social support for carers to reduce feelings of isolation and anxiety. Lymphoedema Service The service will provide a community lymphoedema service which supports the specialist management of individuals living with lymphoedema, lipoedema and chronic oedema aged 18 and above. The service will be delivered in line with British Lymphology Society guidelines.

This holiday provision is for school aged children from reception to year 11 (inclusive) who receive benefits-related free school meals. For 2026 to 2027, the HAF programme will remain in line with the existing FSM threshold, for the whole of the delivery year (April 2026 – March 2027). From September 2026, when the new FSM expansion takes place, HAF will not be expanded to children from households in receipt of Universal Credit for this delivery period. The contract is estimated to commence on 01 October 2026 and will be in place for a period of 2 years and 6 months (end March 2029) with the option to extend for a further 1 year subject to satisfactory performance and funding availability.
