The service provided covers the population of North Lincolnshire. Community Lymphoedema Service The service will provide a community lymphoedema service which supports the specialist management of individuals living with lymphoedema, lipoedema and chronic oedema aged 18 and above. The service will be delivered in line with British Lymphology Society guidelines. The service provided covers the population of North Lincolnshire. Discharge to Assess (D2A) and Community Beds The service will provide six nurse-led community beds provide short-term care for patients who are medically fit for discharge but require further assessment, therapy, or care planning. The beds support timely discharge from Scunthorpe General Hospital and enable patients to receive care in a community setting while longer-term arrangements are made.
