The Bristol Dementia Wellbeing Service delivers an integrated person-centred primary care-led model for people with dementia and their family/carers, at home, in care homes or other residential settings. This service is designed to offer comprehensive support with the primary aim of enhancing the wellbeing and quality of life for those affected by dementia and ensuring a timely diagnosis. The General Practitioner is the co-ordinator of care for people with dementia and their family/carers. The Bristol Dementia Wellbeing Service work alongside, providing a co-ordinated and integrated approach to assessment diagnosis and treatment and are responsive to the requirements of primary care, providing specialist advice, ongoing support, and specialist interventions. The service assigns a dedicated dementia practitioner and advisor to each GP practice, and proactive touch points are maintained with individuals every six months. Holding individuals on its caseload from diagnosis through to end-of-life, The comprehensive model offers diagnostics support, care home support, proactive, in reach support and training plus a portfolio of groups and activities for people and their carers. The service also provides a well-utilised help and advice line The service also offers a wide range of support, including personalised care plans, regular assessments, and access to a variety of services such as occupational therapy, counselling, and social activities. The service also provides valuable information and advice to help individuals and their families navigate the complexities of dementia care.
