REPUBLIC OF THE PHILIPPINES CITY OF LAPU-LAPU 6015 BIDS AND AWARDS COMMITTEE INVITATION TO BID FOR SUPPLY AND DELIVERY OF 3,333 SYRINGE INFLUENZA POLYVALENT VACCINE The City of Lapu-Lapu, through the MOOE FUNDS 4411-5-02-03-070-4 intends to apply the sum of One Million Nine Hundred Ninety-Nine Thousand Eight Hundred Pesos (₱ 1,999,800.00 ) being the Approved Budget for the Contract (ABC) to payments under the contract for the procurement of SUPPLY AN DELIVERY OF 3,333 SYRINGE INFLUENZA POLYVALENT VACCINE under P.R No. 26-08-1103. Bids received in excess of the ABC shall be automatically rejected at bid opening. 1) The City of Lapu-Lapu now invites bids for SUPPLY AND DELIVERY OF 3,333 SYRINGE INFLUENZA POLYVALENT VACCINES. LOT (ONE TIME DELIVERY); 2. IF APPLICABLE, AT LEAST 24 MONTHS EXPIRY UPON DELIVERY; 3. DELIVERY SHALL BE MADE WITHIN 30 CALENDAR DAYS FROM RECEIPT OF THE NOTICE OF AWARDS; AND 4. IF APPLICABLE, THE DELIVERY RECEIPT MUST CONTAIN THE PURCHASE ORDER NO., LOT/BATCH NO., EXPIRY DATE OF EACH ITEM, AND UNIT COST OF EACH ITEM AS REFLECTED IN THE PURCHASE ORDER.
