LOT (ONE TIME DELIVERY); 2. IF APPLICABLE, AT LEAST 24 MONTHS EXPIRY UPON DELIVERY; 3. DELIVERY SHALL BE MADE WITHIN 30 CALENDAR DAYS FROM RECEIPT OF THE NOTICE OF AWARDS; AND 4. IF APPLICABLE, THE DELIVERY RECEIPT MUST CONTAIN THE PURCHASE ORDER NO., LOT/BATCH NO., EXPIRY DATE OF EACH ITEM, AND UNIT COST OF EACH ITEM AS REFLECTED IN THE PURCHASE ORDER. Valid License to Operate (LTO), which will indicate the list of sources of the Companies that a distributor is only to carry and sell/ Authority to Distribute. 3. Valid Certificate of Good Manufacturing Practice (CGMP). 4. Certificate of Distributorship Agreement duly received by the FDA with Reference Number 5.
