Please see the attached RFQ, sealing template, Technical Specifications and updated NGPA Omnibus Sworn Statement template. These documents shall be attached upon submission of the quotation: - PHILGEPS Registration Certificate or Registration Number - Valid Mayor’s/ Business Permit. These documents may be submitted prior to the issuance of the Notice of Award. The winning supplier will be informed by the end-user regarding their submission: - Original Notarized Omnibus Sworn Statement for ABCs above ₱50,000 (updated template) - Business/Income Tax Return for ABCs above ₱500,000.
