Name of the Project : Procurement of Construction Materials for the Improvement of Day Care Center (Completion) at Barangay Camanga, Badoc, Ilocos Norte. Approved Budget for the Contract : Three Hundred Six Thousand Five Hundred Ninety One Pesos Only (306,591.00) Specifications: : See attached Annex “A” Delivery Term : Within Three (3) days from receipt of the Contract Interested suppliers are required to submit their valid and current Mayor’s/Business Permit, Income/Business Tax Return, Philgeps Registration Certificated and duly signed Price quotation form (Annex “A”). Submission of quotation and eligibility of documents at BAC Office, Municipal Hall of Badoc, Ilocos Norte, Quotation and eligibility documents must be submitted in 2 copies each in a sealed envelope. Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above the other terms and condition stated in the price quotation form. Any interlineations, erasures or overwriting shall be valid only if they are signed or initiated by the bidder.
