Republic of the Philippines Province of Cebu Municipality of Cordova BIDS & AWARDS COMMITTEE October 19, 2026 Request for Quotation (RFQ) The Municipality of Cordova, through its Bids and Awards Committee (BAC), is inviting all interested bidders to submit a Quotation for the Purchase of Community Tuberculosis Supplies for 4th Quarter for Cordova Primary Health Care Facility, subject to the submission of the following requirements: 1. Copy of DTI or Sec Registration Certificate; 2. Copy of Current Mayor’s Permit; 3. Copy of Taxpayer’s Identification Number (TIN); 4. Tax Clearance; 5. PhilGEPS Registration Certificate; and 6. Omnibus Sworn Statement. A complete set of Quotation Forms are available at the BAC Office. Deadline for the submission of open proposals together with the documentary requirements is on October 22, 2026, 10:00 AM at the Bids and Awards Committee Office, 2/F Legislative Building, Municipality of Cordova, Cordova, Cebu. The Approved Budget for the Contract is TWO HUNDRED FIFTY THOUSAND THREE HUNDRED EIGHTY-ONE PESOS (PhP250,381.00) only. IMPORTANT TERMS AND CONDITIONS 1. All quotations submitted to the Municipality of Cordova are considered as offer. In the event that the quotation is accepted, a Work Order shall be placed with the contractor offering the most advantageous terms to the Municipality of Cordova. 2. Work is subject to inspection and acceptance. Payment shall be in accordance with the usual budgeting, accounting and auditing requirements; and 3. The Municipality of Cordova reserves the right to accept or reject any or all offers at any time prior to the award of the project without incurring any liability to the affected proponent and to waive any minor defects therein, and to accept such offer it may consider most economical and most advantageous to the Government. 4.
