Please quote your lowest net price, inclusive of Value-Added Tax (VAT) and all other applicable taxes, duties, and charges, for the item(s)/work specified in the attached Request for Quotation (RFQ). Your duly accomplished and signed Request for Quotation (RFQ), together with the required eligibility documents, must be submitted to the BAC Secretariat on: Submission Deadline: October 26, 2026, 9:00 A.M. Venue: BAC Office, Municipal Hall, Casiguran, Sorsogon Required Documents: 1. Duly accomplished and signed Request for Quotation (RFQ); 2. Valid PhilGEPS Registration Certificate; 3. Valid Mayor's/Business Permit 4. Valid BIR Certificate of Registration and Tax Clearance Certificate, where applicable; 5. DTI Certificate of Business Name Registration (for sole proprietorship) or SEC Registration Certificate (for corporations, partnerships, or cooperatives); and 6. Duly signed and notarized Omnibus Sworn Statement. Failure to submit the required documents within the prescribed period may be a ground for the rejection of the quotation.
