INVITATION TO SUBMIT QUOTATION Small Value Procurement SVP 2026-1020-11 Project Name: PROPOSED RENOVATION OF ROOFING AT MAIN HOSPITAL BUILDING [PHASE 1] RFQ No.: SVP 2026-1020-11 Approved Budget of the Contract (ABC): PhP 1,950,000.00 Dear Sir/Ma’am: The Datu Odin Sinsuat District Hospital through its Bids and Awards Committee (BAC) invites interested suppliers to submit their quotations/proposals for the project Proposed Renovation of Roofing at Main Hospital Building [Phase 1] located at Main Hospital Building of the hospital in accordance with Section 34 – Small Value Procurement of the 2024 Implementing Rules and Regulations (IRR) of Republic Act No. 12009. The quotation must be accompanied by the following mandatory requirements in a sealed envelope: • Valid PhilGEPS Registration Number/Certificate (Platinum or Red); • Mayor’s/Business Permit; • Income/Business Tax Return; • Notarized Omnibus Sworn Statement (OSS); • Notarized Authorization (if signatory is executed by its duly authorized representative). Sealed quotations must be submitted on or before 03:00 PM of October 30, 2026 at the Conference Room (2nd Floor) – Main Hospital Bldg., DOSDH Compound, Dalican Poblacion, Datu Odin Sinsuat, Maguindanao del Norte; or send via email to our BAC Secretariat: sandrabalabaran@gmail.com. Late bids shall not be accepted. Quotations exceeding the Approved Budget for the Contract (ABC) shall be automatically rejected. Price must be inclusive of VAT and all applicable taxes. The Head of Procuring Entity at DOSDH reserves the right to accept or reject any quotation, and to annul the process at any time prior to contract award, without incurring any liability to the affected quoter(s).
