These documents shall be attached upon submission of the quotation: - Updated PHILGEPS Registration Certificate. - Valid Mayor’s/ Business Permit. These documents may be submitted prior to the issuance of the Notice of Award - Original Notarized Omnibus Sworn Statement for ABCs above ₱50,000 (updated template) - Business/Income Tax Return for ABCs above ₱500,000.
