Name of Project : "Supply and Delivery of Construction Materials for the Construction of Post Harvest Facility Building (Concreting of Flooring, Construction of Platform and Stairs and Installation of Power Supply and High Bay Lights)" Approved Budget for : Two Hundred Fifty Six Thousand Eight Hundred Ninety Six Pesos Only The Contract (ABC) (Php256,896.00) Specifications : See attached Annex “A” for Specifications Location : Victoria, Oriental Mindoro Delivery Term : Within Ten (10) c.d. upon receipt of Notice to Proceed Interested Suppliers are required to submit their valid and current Mayor’s Permit, Latest Income and Business Tax Return, PhilGEPS Registration Number, Omnibus Sworn Statement, and price quotation form (Annex “A”) during submission of offer/quotation. Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form. Any interlineations, erasures, or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s.
