Please quote your lowest and most advantageous net price, inclusive of Value-Added Tax (VAT), if applicable, and all other applicable taxes, duties, fees, and charges, for the infrastructure project specified in the attached Request for Quotation (RFQ) and/or Bill of Quantities (BOQ). Your duly accomplished and signed RFQ and BOQ, together with the applicable documentary requirements, shall be submitted to the Bids and Awards Committee (BAC) Secretariat at the following address, on or before the prescribed deadline: Submission Deadline: November 3, 2026, at 9:00 AM Venue: BAC Office, Municipal Hall, Casiguran, Sorsogon REQUIRED DOCUMENTARY REQUIREMENTS The following documents shall be submitted, as applicable under the procurement rules governing the project: 1. Duly accomplished and signed Request for Quotation (RFQ) and Bill of Quantities (BOQ); 2. Valid PhilGEPS Registration Certificate or applicable PhilGEPS registration document; 3. Valid Mayor’s/Business Permit or its equivalent document; 4. Valid BIR Certificate of Registration and Tax Clearance, where required by applicable procurement rules; 5. DTI Certificate of Business Name Registration for sole proprietorships, or SEC Registration Certificate for corporations, partnerships, or other applicable juridical entities; 6. Valid Philippine Contractors Accreditation Board (PCAB) License and registration appropriate to the nature and classification of the infrastructure project, where required by law; 7. Duly accomplished and signed Omnibus Sworn Statement (OSS), notarized in accordance with applicable procurement requirements; and 8. Other documentary requirements expressly required under the applicable procurement rules and regulations.
