The support is based on a recovery and rehabilitation focused model, with all service users receiving a core package of support from a staff group. Support received will be delivered as stipulated in each service users Social Care Assessment and individual Care Plan. Each service user will also require an outcome planner to evidence their support/ rehabilitation package which enables them to gain the skills required to live independently in the community. It is expected that the successful provider will work closely with Adult Social Care and Commissioning to support reintegration into the wider community. The service will provide support in addition to the existing support delivered under the Community Mental Health Framework (CMHF). It does not replace existing commissioned services.

This approach recognises the critical role that lived experience plays in improving service relevance, quality, engagement, retention, and recovery outcomes. The commissioned service will operate independently, while working collaboratively with the wider substance use treatment system, to strengthen recovery capital, amplify lived experience voices, and support sustained recovery within our local communities.

The estimated dates between which this service was intended to be provided was for 12 months with the option to extend for 2 x 12 months (i.e., a one-year agreement that can be extended for two additional, separate one-year periods, if both parties agree). 3 years total contract length up to 20 July 2029.

North Northamptonshire Council (NNC) is re-opening their Dynamic Purchasing System (DPS) for the supply of services in respect of NHS Health Checks. The successful Potential Provider(s) will be responsible for providing this service and liaising closely with the Contract Manager identified by The Council. The NHS Health Check programme aims to help prevent heart disease, strokes, diabetes, kidney disease and certain types of dementia. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions or have certain risk factors, will be invited (once every five years) to have an NHS Health Check to assess their risk of heart disease, stroke, kidney disease and diabetes; and will be given support and advice to help them reduce or manage that risk. Please note: this contract originally started on 1st April 2024. North Northamptonshire Council currently commission an NHS Health Check programme that has been successful to an extent, and there is an ambition to build on this success by providing a wider and more successful programme to the localities across the North Northamptonshire Council footprint.

End of Life and Palliative Care Services - Daycare The service will provide access to specialist end of life and palliative care daycare services which will include assessment, therapy, counselling and social support. The service provided covers the population of North Lincolnshire. Community Lymphoedema Service The service will provide a community lymphoedema service which supports the specialist management of individuals living with lymphoedema, lipoedema and chronic oedema aged 18 and above. The service will be delivered in line with British Lymphology Society guidelines. The service provided covers the population of North Lincolnshire. Discharge to Assess (D2A) and Community Beds The service will provide six nurse-led community beds provide short-term care for patients who are medically fit for discharge but require further assessment, therapy, or care planning. The beds support timely discharge from Scunthorpe General Hospital and enable patients to receive care in a community setting while longer-term arrangements are made.

Central Referral Mechanism and Support Service for Adult Victims, establishing a core element of its new London-wide Ecosystem of victim support services. The provider will be responsible for: Managing police referrals to ensure victims are swiftly directed to the appropriate specialist services; Assessing the needs of adult victims of crime; Delivering a range of practical and emotional support; Working collaboratively with partners across the Ecosystem to enhance service quality and outcomes; Maintaining the LVWS website and service directory; Providing pre-trial information and support to all court witnesses, including defence witnesses; Coordinating with relevant agencies to respond effectively to major crime incidents; Adapting services in line with changing demand and developments within the criminal justice system. A Competitive Procedure with Negotiation under the Light Touch Regime, pursuant to the PCR 2015 was undertaken. Due to issues with FTS we have been unable to link the Contract Notice to this Award Notice which has restricted publication under an Competitive Procedure. This contract has an initial term of 4 years, commencing on 01/04/2024 - 31/03/2028 at a value of £15,400,000 with two options to extend for one year each at a value of £4,400,000 per annum, exercised at the discretion of MOPAC. There is an additional £17,660,000 available to provide additional services which may arise over the contract life, should funding become available. Total available budget for this contract is £41,860,000. Additional Variations to this Contract, to date, include the following: Approach to Developing Ecosystem's online presence at a value of £90,645. Following an initial review of the updated webpages, supported by digital specialists and key stakeholders, it has been identified that further enhancements are required to deliver a more seamless and accessible experience for victims and witnesses. The Restorative Justice Service for London at a value of £400,000 which will sit as part of the wider Universal Adult Support Service. London DRIVE Project at a value of £865,312 to support the additional delivery of Drive victim support. The inclusion of these variations falls far below the total contract value of £41,860,000.
