In Greater Manchester there are an estimated 120,000 people experiencing Multiple Disadvantage. This is defined as a combination of homelessness, substance misuse, mental health issues, domestic abuse and contact with the criminal justice system. People facing multiple disadvantage are at the sharp end of socio-economic and health inequalities. As part of Greater Manchester's Live Well commitment to tackling health inequality, we want to build on the existing Inclusion health service to reduce this inequality. GMCA are looking for providers to deliver three distinct services which will remove a range of different barriers for people experiencing MD to having their health needs met. Lot 1: Co-occurring Conditions Support- Delivery support to people experiencing Co-occurring conditions across GM, through clinical and non-clinical roles integrated within enhanced live well teams. Including providing specialist support to professionals and delivering share reflective practice spaces. Total estimated value over 3 years is £4,839,375.16 Lot 2: Primary Care Led Support- Deliver support in a primary care setting to people facing Multiple Disadvantage, in Rochdale, Oldham and Manchester. This high-quality health and social support will be embedded in primary care and work collaboratively with other agencies to improve public health outcomes. Total estimated value over 3 years is £1,500,000 Lot 3: Palliative Care Support- The provision of a palliative care service for adults experiencing homelessness alongside life-limiting and end-of-life physical health conditions. Total estimated value over 3 years is £300,000
