Description of Service The Health Access Worker service will improve access to healthcare for people sleeping rough by providing wraparound connections for rough sleeping clients into the health system. The service will provide lived experience as a core component of provision. Be headquartered at the Outreach provider’s office, but with the understanding that it is peripatetic/out in the field for a primary portion of the workday.
